CPM, CNM, Doula – What’s the Difference?

Lead author, Massachusetts Community Birth and Doula Initiative

An explanation of the differences between different types of midwives and doulas.

This Community Birth 101 resource clarifies the distinct but complementary roles of certified professional midwives (CPMs), certified nurse-midwives (CNMs), and doulas. CPMs and CNMs are clinical providers offering preventive, reproductive, pregnancy, birth, newborn, and postpartum care; doulas provide nonclinical emotional, physical, and informational support. It compares their education, certification, practice settings, and Massachusetts policies. CNMs have practiced independently since 2012, while CPM licensure and MassHealth eligibility were mandated in 2024, with regulations developing. A state doula certification program launched in 2026. Together, these professionals strengthen perinatal systems by centering relationships, community, culture, lived experience, advocacy, and coordinated care for families.
