Resource: Toolkit
AIM Community Birth Transfer Resource Kit
This kit strengthens community birth transfers through teamwork, protocols, drills, equity, and quality improvement.
The Community Birth Transfer Resource Kit offers a practical framework for improving transfers from planned home births and freestanding birth centers to hospitals. Developed for multidisciplinary quality-improvement teams, it emphasizes readiness, recognition and prevention, response, reporting and systems learning, and respectful, equitable, supportive care. The kit recommends collaborative workgroups, standardized protocols, clear communication, shared education, drills, simulations, huddles, debriefs, data collection, and protected case review. It centers patient autonomy, informed decision-making, trauma-informed care, cultural humility, and family involvement. Designed for adaptation to local contexts, the resource helps community providers, hospitals, EMS, doulas, and families build safer, more coordinated transitions together.
